The card informs the user that they have the right to use the card only if, at the time of receiving health care, the conditions under which the card was issued still apply. The user is responsible for any health care costs incurred due to misuse of the card. The user is required to use the card together with an identification document proving their identity. The cardholder must notify the associated labor organization in which they work, or the community, of any changes affecting the card’s details or its issuance (such as termination of work or activity, regular schooling, or dependent status) within 8 days. The health care organization (OUR) may provide health care to the cardholder only based on a validated card and a valid identification document, except in emergency cases. For outpatient specialist and inpatient health care, a referral from a primary health care physician from the user’s community is also required. The primary health care OUR sends the claim (invoice) for the health care costs of the employed cardholder to the OUR responsible for their contributions. In all other cases of using health care, the health care OUR sends the claim (invoice) to the community that issued the card.
